The file set as Cover position in the Item Images & Attachments Panel is shown as the thumbnail of an Album/Item in the Photo Gallery Anatomy.

Only one file can be set on this position and it will not be displayed explicitly in the album.

The Cover image can also be used for various other purposes, especially for social media meta data.
